CN I - Olfactory Nerve tests

  • Observation of nose

  • Test each side with different scent

  • Internal nose exam

CN II - Optic Nerve tests

  • Observation of the eyes

  • Visual acuity (w/Snellen’s chart)

  • Confrontation/Peripheral vision

  • Pupillary light reflex

  • Opthalmascopic/Internal eye exam

CN III, IV, VI - Oculomotor, Trochlear, Abducens nerve tests

  • Observation of eyes

  • Corneal light reflex

  • Pupillary light reflex

  • 6 Cardinal Fields

  • Accommodation

CN V - Trigeminal nerve tests

  • Muscles of mastication

  • Corneal blink reflex

  • Sensory exam of face

  • Jaw jerk reflex

CN VII - Facial nerve tests

  • Facial muscles (smile, frown, look up, wrinkle forehead)

  • Taste of anterior 2/3 of tongue

CN VIII - Vestibulocochlear nerve tests

  • Observation of external ears

  • Weber & Rinne tests

  • Otoscopic exam

  • Caloric irrigation (COWS)

  • Swivel chair test

  • Rotating head for dizziness

CN IX, X - Glossopharyngeal, Vagus nerve tests

  • “Ahhh” (soft palate elevation)

  • Gag reflex (post 1/3 tongue)

  • Phonation ( Kuh-La-Mi)

  • Taste (post 1/3 tongue)

CN XI - Spinal Accessory nerve tests

  • Shoulder elevation

  • Muscle strength (SCM, Trap)

CN XII - Hypoglossal nerve tests

  • Stick out tongue

  • Tongue in cheek

Cervical ROM: 50º flexion, 60º extension, 80º rotation

Thoracic ROM: 50º flexion, 30º rotation

Lumbar ROM: 60º flexion, 25º extension, 25º rotation

  • Deltoid — C5, C6 — Axillary n

  • Biceps — C5, C6 — Musculocutaneous n
