Overview of Brain Stem Physiology

  • The brain stem consists of three main parts: medulla oblongata, pons, and midbrain.

  • Central nervous system is composed of the brain and spinal cord, formed by neurons and supportive cells (neuroglia).

  • Gray Matter vs. White Matter:

    • Gray matter: contains nerve cell bodies and proximal parts of nerve fibers.

    • White matter: consists mainly of myelinated axons.

    • In spinal cord: white matter is on the outside, gray matter is on the inside.

    • In the brain: gray matter is on the outside, white matter is on the inside.

Main Divisions of the Brain

  • Three Major Divisions:

    1. Prosencephalon (Forebrain)

    2. Mesencephalon (Midbrain)

    3. Rhombencephalon (Hindbrain)

  • Further divisions include:

    • Prosencephalon: Telencephalon (cerebral hemispheres, basal ganglia) and Diencephalon (thalamus, hypothalamus, etc.)

    • Rhombencephalon: Metencephalon (pons, cerebellum) and Myelencephalon (medulla oblongata)

Medulla Oblongata

  • Originates from myelencephalon, is an extension of the spinal cord.

  • Contains two significant parts: rostral (fourth ventricle) and caudal (central canal).

  • Functions of Medulla Oblongata:

    • Conduction: Main pathway for ascending and descending tracts of the spinal cord.

    • Reflector: Essential reflex actions and vital centers.

    • Centres located in Medulla:

    • Cardiovascular centers (heart rate, blood pressure regulation).

    • Respiratory rhythmicity center (controls breathing rhythm).

    • Centers for digestion, protective reactions (vomiting, sneezing).

Cranial Nerve Nuclei in Medulla

  • Houses nuclei for several cranial nerves (CN):

    • 12th Cranial Nerve (Hypoglossal): Controls tongue movement.

    • 11th Cranial Nerve (Accessory): Controls shoulder movements.

    • 10th Cranial Nerve (Vagus): Regulates vital functions (cardiovascular, respiratory, digestive).

    • Vestibulocochlear (CN VIII): Processes auditory and equilibrium information.

Pons

  • Functions as a bridge between medulla and midbrain.

  • Major Functions include:

    • Connecting cerebellum to higher brain centers.

    • Passageway for pyramidal tracts.

    • Houses nuclei of cranial nerves (V, VI, VII & VIII).

    • Contains pneumotaxic and apneustic centers regulating respiration.

Midbrain

  • Located between diencephalon and pons, divided into:

    • Tectum: Includes superior/inferior colliculi (visual/auditory processing).

    • Tegmentum: Floor of midbrain, contains reticular formation, red nucleus, and substantia nigra.

  • Superior Colliculus: Involved in head, eye, and limb movement in response to visual stimuli.

  • Inferior Colliculus: Processes auditory information, controls orientation of movements to sound.

Cerebral Peduncles in Midbrain

  • Composed of:

    1. Basis Pedunculi: Houses pyramidal tract fibers.

    2. Substantia Nigra: Involved in motor control, part of basal ganglia.

    3. Tegmentum: Contains various nuclei involved in motor functions.

Red Nucleus Functions

  • Plays a pivotal role in:

    • Control of muscle tone and complex muscular movements.

    • Processing and coordinating righting reflexes (to maintain posture).

    • Ocular movement control via connections with cranial nerve nuclei.

    • Involvement in skilled motor tasks through spinal cord and cerebral cortex interactions.
