Catheterization Check-off
Performs hand hygiene, dons PPE if necessary.
Hello Mr./Ms. Smith, My name is Angie and I will be your student nurse today. Today I will be inserting your catheter.
Can I please get your full name and date of birth?
I am now performing a safety room check. I am looking at the ambulatory bag, oxygenation, nasal cannula, suction, Christmas tree, non-rebreather, and EKG machine. I checked it and it is all functioning properly.
I am verifying the practitioner’s order on the chart, and it says here that you are in need of a catheter due to acute urinary retention issue. I am checking for your medical record for allergies. There is no indication. Do you have any allergies?
I will put the bed in appropriate working height.
Apply blue gloves.
Froggy position. Performs perineal care.
Discard gloves and perform hand hygiene.
Open catheter kit, flaps away from the body first.
Applies sterile gloves before beginning of the procedure. Can be on a different surface
Prepare are with protector: shiny side down and protector over the peri area.
Clean the peri area with antiseptic swabs. 3x switch each time.
Prepare the lube. Put the lube on the catheter.
Attach the syringe fille with sterile water to the balloon.
Female:
15. Cleans labia with wipes first, starting from each side on the outside, and ends with the center of the labia.
16. Clean labia with antiseptic swabs. 3x switch each time.
17. Please relax and take a deep breath. I will insert the catheter in the urinary meatus until urine is visualized. I then advance the catheter about 2-3 inches further until I feel resistance.
18. I will the inflate the balloon. I gently tug on the catheter is snug against the bladder beck. I will then secure the catheter to the pt’s thigh using securement device or tape.
Male:
16. Clean the penis with wipes first, going in circular motion from middle to outwards.
17. Clean penis with antiseptic swabs. 3x switch each time from middle to outwards.
18. Please relax and take a deep breath. Slowly insert the catheter tip into the penile shaft until urine is visualized and keep going until it reaches bifurcation. I will then advance about 2-3 inches further.
19. I will then inflate the balloon using the syringe. I will tug until the inflated balloon is snug against the bladder neck.
20. I will then secure catheter to the pt’s thigh using securement device or tape.
Closing:
21. Okay you are all done. I have removed all of the equipment.
22. I will leave the call light in reach. If you need anything please press your call light. Bedside table, assistive devices, and belongings in reach. Side rails are up if ordered. Bed in low and locked position. Before I leave, is there anything else I can do for you?
23. Remove PPE. Hand Hygiene.
24. I will then document that care is performed, I will document date and time, type of insertion, used sterile technique, pt tolerated procedure well with no complaints, urine is pale and yellow, no abnormalities and procedure were done once with no complications.
