Definition

  • Well-differentiated neuroendocrine tumors, commonly referred to as carcinoid tumors, arise from neuroendocrine cells.
  • These tumors are characterized by:
    • Uniform tumor cells with round nuclei
    • Salt and pepper chromatin
    • Eosinophilic cytoplasm

Presentation

  • Although most carcinoid tumors are found incidentally and are asymptomatic, some can obstruct bodily lumens, potentially leading to acute medical conditions, such as acute appendicitis.

Adenoma to Carcinoma Sequence in Colon Cancer

Overview

  • The progression from adenoma to carcinoma involves multiple steps characterized by genetic alterations and increased cellular dysplasia.

Pathophysiology

  1. Normal Colon
    • Starts with normal epithelial cells.
  2. Hyperproliferative Epithelium
    • Characterized by increased cell division.
    • APC Inactivation
      • Affected by inactivation of the APC (Adenomatous Polyposis Coli) gene.
  3. Adenoma Development
    • Progression leads to the formation of adenomatous polyps.
    • DCC (Deleted in Colorectal Carcinoma) inactivation.
    • K-Ras activation
      • Oncogenic mutation leading to further dysregulation.
    • Increased methylation changes.
  4. Carcinoma
    • Further genetic abnormalities accumulate, leading to carcinoma development, characterized by:
      • Abnormalities in p53 gene,
      • Overexpression and activation of oncogenes, and inactivation of tumor suppressor genes.

Characteristics of Adenomatous Polyps

  • Adenomatous polyps are defined by the presence of dysplastic mucosa, making them premalignant lesions.
  • Impact of Regular Screening:
    • Effective prevention of colon adenocarcinoma is achievable through regular screening and timely excision of polyps.
    • Increased activity of cyclooxygenase-2 (COX-2) has been linked to specific forms of colon adenocarcinoma.
    • Aspirin Use
    • Regular aspirin consumption has been suggested to decrease the formation of adenomatous polyps.
