Evaluation in continuous
Evaluation is done while or immediately after implementing a nursing order to enable the nurse to make on-the-spot modification of the intervention.
Evaluation at specified intervals shows the extent of progress towards goal achievement and enables the nurse to correct any deficiencies and modify the care plan needed.
