CPT code 65710, keratoplasty, is the surgical removal of a damaged or diseased portion of the cornea, followed by the implantation of healthy tissue from a donated human cornea.

CPT code 65760, keratomileusis, is the surgical reshaping of the cornea, carried out in order to correct a refractive error.

CPT code 65765, keratophakia uses a human donor cornea (lenticule) to produce an increase in the anterior curvature of the recipient cornea

CPT code 65770, keratoprosthesis, is a surgery to implant an artificial cornea.


CPT codes 99401–99404 and 99411–99412 are used for patients who are not obese. These are preventive medicine codes for patients that do not have a specific illness. During the counseling session the provider will consult with the patient regarding diet, exercise, and obesity and provide behavior change interventions. CPT codes 99401–99404 are for individual counseling based on time. The correct answer is CPT code 99401, which is for 8–22 minutes. CPT code 99402 is for 23-37 minutes.


Modifiers are found in Appendix A of the CPT code book. When a service has a technical and professional component performed in the hospital. The physician can only use modifier 26 for the professional component.
