Intellectual Disabilities
Definition: Evident in childhood; significantly low intellectual and adaptive functioning.
Note: Referred to as Intellectual Developmental Disorder in DSM-5 TR.
Diagnostic Criteria:
A. Deficits in intellectual functions confirmed by clinical assessment and standardized intelligence testing.
B. Deficits in adaptive functioning leading to failure to meet developmental and sociocultural standards.
C. Onset of deficits during developmental period.
Severity Specifiers & Prevalence
Severity Categories:
Mild (85% prevalence): Able to live independently with minimal support.
Moderate (10%): Moderate level of support required for independent living.
Severe (3.5%): Daily assistance needed for self-care.
Profound (1.5%): Requires 24-hour care.
Prevalence:
~1% of the general population; 6 per 1,000 for severe intellectual disability.
Risk Factors:
Environmental (abuse, neglect)
Prenatal (exposure to drugs)
Perinatal (labor complications)
Postnatal (infections, head injuries)
Differential Diagnosis of Intellectual Disability
Major/mild neurocognitive disorders.
Communication disorders and specific learning disorders may co-occur but must meet all ID criteria.
Autism Spectrum Disorder may be present with ID as well.
Communication Disorders
Language Disorder: Struggling with expressing and understanding language.
Diagnostic Criteria for Language Disorder:
Persistent difficulties in acquiring and using language.
Abilities below age expectations causing limitations in communication, participation, or achievement.
Onset in early development.
Not due to other impairments (hearing, motor, etc.).
Risk Factors for Language Disorders
Receptive impairments lead to poorer outcomes than expressive impairments.
Language disorders have a hereditary component.
Differential Diagnosis for Language Disorder
Normal variations in language.
Hearing impairments must be ruled out.
Mixed intellectual disabilities; ensure language deficits exceed overall intellectual limitations.
Speech Sound Disorder
Definition: Persistent difficulty in producing speech sounds impacting communication.
Diagnostic Criteria:
Difficulty with sound production affecting speech intelligibility.
Impairs social, academic, or occupational performance.
Early developmental onset.
Not due to other health conditions.
Differential Diagnosis for Speech Sound Disorder
Hearing impairments.
Dysarthria related to motor disorders (e.g., Cerebral Palsy).
Selective Mutism as a potential comorbid condition.
Childhood-Onset Fluency Disorder (Stuttering)
Definition: Disruption in normal speech flow.
Diagnostic Criteria:
Disturbances in fluency and timing pattern of speech.
Symptoms cause speaking anxiety and limitations in communication.
Onset in early development.
Not due to sensory or motor deficits, neurological insults, or other mental disorders.
Social (Pragmatic) Communication Disorder
Definition: Persistent difficulties with verbal and nonverbal communication used in social contexts.
Diagnostic Criteria:
Problems using communication for social purposes.
Impairments in adjusting communication based on context.
Difficulties following conversational rules.
Trouble understanding implied meanings.
Autism Spectrum Disorder (ASD)
Definition: A neurodevelopmental disorder that impacts perception and social interaction.
Diagnostic Criteria:
Persistent deficits in social communication and interaction across settings.
Restricted/repetitive patterns of behavior and interests.
Symptoms present early in development.
Significant impairment in functioning.
Not better explained by other disorders.
Attention-Deficit/Hyperactivity Disorder (ADHD)
Definition: A behavioral condition characterized by inattention and/or hyperactivity-impulsivity.
Diagnostic Criteria:
Pattern of inattention and/or hyperactivity-impulsivity affecting functioning (specific symptoms listed).
Symptoms present prior to age 12.
Observed in two or more settings.
Specific Learning Disorder
Definition: Performance below expected levels in academic skills.
Diagnostic Criteria:
Persistent difficulties in academic skills despite interventions.
Academic skills significantly below expected levels.
Learning issues begin during school-age years.
Not due to other identifiable factors.
