Postpartum mom

Norm: Breasts 1-2 day breast and nipps are soft, intact, secreting colostrum.

Not norm: No tenderness, head, edema (engorment). Reddened area (mastitis). Palpable mass (mastitits). Engorgement (venous stasis)

Nipples

Norm: Pigmented, intact, erect when stimulated

Not norm: Invereted nipples = no erect when stimulated. Fissures, cracks, sorneess = poor breast feeding techniques

Fundus every 15 min for 1 hr, every 8 hr for 48 hr

Norm: Firm in midline and atslightlyu above umbilicus

Not norm: Boggy fundus (massage until firm, report Healthcare provider, observe, empty bladder, medicate with oxytocin). Fungus is 1-2 fingerbreadths from midle means full bladder (pt must void or cath)

Lochia every 15 min for 1 hr, every 8 hr for 48hrs

Norm: Scant to moderate amount, earthy odor, no clots (3 days postpartum rubra, mod amount and steadly decreases, odor minimal)

Not norm: Clots indicate hemorrhage

Perineum

Norm: slight edema & bruising

Not norm: sweeling bursing = hematoma

Episiotmy

Norm: intact/no swelling/no discoloration.

Not norm: Redness, ecchymosis (bruising), discharge, gaping stiches = infection

Hemorrhoids

Norm: none/few small nontender

Not norm: full, tender, red=inflmationm
