Reason for Referral:Carson was referred by his parents to assess his abilities and to understand his difficulties with anxiety, inattention, and inconsistent academic performance.
Background Information
Developmental History:
Healthy pregnancy; born at 37 weeks, had brief NICU stay for fluid in lungs.
Attained developmental milestones on schedule; attended daycare and preschool.
Noted attention weaknesses by preschool teacher but sent to kindergarten.
Current Education:Carson is currently in kindergarten, receiving small group intervention for early reading and writing skills. He expresses anxiety about his academic performance and has difficulties with organization and task follow-through at home.
Family Structure:Lives with both parents and an older brother. Family history of ADHD and anxiety noted.
