BDI Question Examples
Sadness:
0: I do not feel sad.
1: I feel sad.
2: I am sad all the time and I can't snap out of it.
3: I am so sad and unhappy that I can't stand it.
Discouragement:
0: I am not particularly discouraged about the future.
1: I feel discouraged about the future.
2: I feel I have nothing to look forward to.
3: I feel the future is hopeless.
Feelings of Failure:
0: I do not feel like a failure.
1: I feel I have failed more than the average person.
2: I see a lot of failures in my life.
3: I feel I am a complete failure.
Satisfaction:
0: I get as much satisfaction out of things as I used to.
1: I don’t enjoy things as much.
2: I don’t get satisfaction from anything anymore.
3: I am dissatisfied with everything.
Guilt:
0: I don’t feel particularly guilty.
1: I feel guilty a good part of the time.
2: I feel guilty most of the time.

Disappointment in Self:

PANSS Rating Instructions
Understanding Ratings:
1: Absent
2: Minimal
3: Mild
4: Moderate
5: Moderate Severe
6: Severe
7: Extreme
Rating Guidelines:
Assess presence of symptom, score accordingly.
Highest applicable score should be assigned.
Holistic perspective to characterize function.
Severity Levels:
2 (Minimal): Subtle pathology.
3 (Mild): Interferes little in daily life.
4 (Moderate): Serious but occasional.
5 (Moderate Severe): Marked impact but containable.
6 (Severe): Frequently disruptive.
7 (Extreme): Drastically interferes with life.
GAD-7 Scale Questions
Questions about anxiety experiences over the past 2 weeks:
Feeling nervous, anxious, or on edge.
Not being able to stop or control worrying.
Worrying too much about things.
Trouble relaxing.
Restlessness.
Easily annoyed or irritable.
Feeling afraid as if something awful might happen.
Scoring: Add column scores for total score.
Work Impact: Assess difficulty in work and home life.
