Vital Signs

Heart Rate

  • Equipment: stopwatch, stethoscope, HR monitor.

  • Procedure (in the absence of a HR monitor): - Using the middle (long) and index (pointer) fingers, gently feel for the radial artery.

    • Count the number of beats for 15 seconds and multiply it by 4 to get beats per minute.

Blood Pressure

  • Equipment: Mercury or aneroid sphygmomanometer, air bladder containing cuff, stethoscope.

  • Procedure: - Subject seated for at least 5 min.

    • Place cuff directly above brachial artery, 1 inch above antecubital space.

    • Clients palm up, feet flat on the floor, and stethoscope placed on antecubital space.

    • Mercury column at eye level.

    • Inflate air bladder to:

    1. 160 mmHg OR

    2. 20 mmHg above expected BP.

    • Release pressure at 2-3 mmHg per second.

    • First audible Korotkoff sound = SBP, disappearance of Korotkoff sounds = DBP.

    • BP recorded in even numbers.

    • Completely release pressure.

    • Take consecutive measurements ensuring that SBP and DBP do not differ by more than 5 mmHg.

    • Allow 2 min between measurements.

Common Errors for Blood Pressure Measurement (Figure 11.2)

  1. The stethoscope is put on backward resulting in the earpieces not pointing toward the personal trainer's ear canals.

  2. The measurement dial is not at the personal trainer's eye level.

  3. The bell of the stethoscope is under the blood pressure cuff.

  4. The blood pressure cuff is placed too close to the bend in the client's elbow (the antecubital space or fossa).

  5. The blood pressure cuff is either too large or small for the client's arm.

  6. The blood pressure cuff is not inflated enough to correctly capture the first Korotkoff sound.

  7. The air is released out of the blood pressure cuff either too quickly or too slowly.

Factors Affecting Blood Pressure (BP) and Heart Rate (HR)

  • Tobacco: Increases resting HR and BP; increases or statically maintains exercise HR.

  • Caffeine: Increases or maintains resting and exercise HR and BP.

  • Extreme Temperature: Increases resting and exercise HR in hot environmental temperatures.

  • Altitude: Increases HR at altitudes greater than approximately 4,000 feet.

  • Stress: Increases resting and exercise HR and BP.

  • Digestion: Increases resting and exercise HR.

  • Body Position: - Decreases HR and BP when supine.

    • Increases HR and BP from supine to seated position or standing position.

  • Time of Day: - Decreases HR and BP first thing in the morning.

    • Increases or maintains during afternoon or evening hours.

  • Medications: Variable effects on HR and BP.

Training Load
  • Definition: Training load reflects the relative balance between various stressors imposed on the body via exercise.- External:

    • Training volume: - For resistance training: sets x reps x load

      • For endurance sports: total distance or number and intensity of training sessions.

    • Internal:

    • The body’s physiological responses to exercise.

    • Rating of perceived exertion (RPE) multiplied by the duration for endurance training or by the total number of repetitions for resistance training.

    Wellness Inventory for Fatigue (Figure 11.5)

    • Assesses sleep quality, muscle soreness, stress level, and fatigue level. Each dimension rated from poor to very good.

    Body Composition

    Anthropometry

    • Definition: Measurement of height, weight, and body circumferences.

    • Height: - Remove footwear and stand as upright as possible against a wall, looking straight ahead.

      • Take a deep breath just before the measurement is taken.

    • Weight: - Wearing minimal clothing, step onto a calibrated scale.

      • Most reliable results occur in the morning after elimination and before the ingestion of food or fluids.

Body Mass Index (BMI)

  • Definition: A formula to categorize individuals based on body weight relative to height. - Note: Does not take into account the relative composition of an individual (fat vs muscle). Recommended for sedentary individuals.

Skinfolds (SF)

  • Definition: A method that indirectly measures the thickness of subcutaneous fat tissue. - High correlation with body density from underwater weighing.
    - For accurate results, use equations that reflect the client’s representative population.
    - Equipment needed: skinfold caliper, tape measure, pen or marking device.

Skinfold Procedures

  • Selecting skinfold sites:

    - Based on body density equations in Table 11.8. 
    - Chest 
    - Midaxillary 
    - Triceps 
    - Subscapular

    • General considerations:

      • Measurements taken on the right side of the body.

      • No lotions or wet skin.

Example: Body Composition Measurements

  • Chest: Diagonal fold halfway between anterior axillary line and nipple for men; 1/3 of the way for women.

  • Mid-axilla: Vertical fold on the mid-axillary line at the level of the xiphoid process.

  • Triceps: Vertical fold on the posterior midline of the upper arm, halfway between top of shoulder and elbow.

  • Subscapula: Diagonal fold from vertebral border to 1-2 cm from inferior angle of scapula.

  • Abdomen: Vertical fold, 2 cm from umbilicus.

  • Suprailium: Diagonal fold above the crest of the ilium at an imaginary line from the anterior axillary line.

  • Thigh: Vertical fold on anterior aspect of the thigh midway between hip and knee.

  • Medial Calf: Vertical fold at the greatest girth of the lower leg, leg at 90° with no weight placed on the leg.

Bioelectrical Impedance (BIA)

  • Definition: A non-invasive, relatively inexpensive method for measuring body composition. - Measures electrical impedance or resistance to a small electrical current passed between two electrodes.
    - Fat is a poor conductor of electricity.
    - Measures total body water (TBW), fat mass, and fat-free mass.
    - Not recommended for clinical populations, especially those with abnormal fluid distributions.

Limitations of BIA

  • Tends to overestimate body fat in lean individuals and underestimate in obese individuals.

  • Factors affecting BIA outcomes: fat distribution, ethnicity, age, sex, body temperature, environmental temperature, hydration status, eating, and drinking.

Note on Body Fat Estimates

  • When assessing client's body composition, account for a standard error of the estimate (SEE) which should be reported as ranges. Example: if a 25-year-old male client’s body fat is measured at 24%, there is a range of 21%-27%. This covering ensures clarity in reporting and reduces ambiguity regarding body fat classification.

Waist-to-Hip Ratio

  • Description: A valuable tool for assessing relative fat distribution and risk of disease.

  • Equipment: Tape measure.

  • Procedure: - Measure the smallest girth of the abdomen.
    - Measure the largest girth of the buttocks.
    - Divide waist size by hip size.

Cardiovascular Endurance

General Procedures

  1. Check the Equipment:

    • Ensure equipment is recently calibrated.

    • Adjust seat height so knee flexion is ~5% at max leg extension.

  2. Instruct your client + Warm-up:

    • Maintain a seated position with the same hand placement throughout.

    • Establish cadence before resistance is applied.

    • Set metronome to twice the desired cadence.

  3. Set Resistance:

    • Set the workload (power output). \frac{\text{kg/m/min}}{6.12}

  4. Monitor Test:

    • Monitor Client appearance for signs and symptoms.

    • Measure variables according to selected tests (HR, BP, RPE, time, distance, etc).

    • Ideal: measure BP and RPE upon stage completion.

  5. Cool Down:

    • Ensure an active or passive cool-down.

    • Monitor vitals for at least 4-min during cool down.

Indications to Stop the Test

General indications for stopping an exercise test:

  • Onset of angina or angina-like symptoms.

  • Drop in systolic BP of ≥10 mmHg with an increase in workload or if SBP decreases below the value obtained in the same position prior to testing.

  • Excessive rise in BP: systolic pressure >250 mmHg and/or diastolic pressure >115 mmHg.

  • Shortness of breath, wheezing, leg cramps, or claudication indicate test termination.

  • Signs of poor perfusion (e.g., light-headedness, confusion, ataxia, pallor, cyanosis, cold or clammy skin, or nausea).

  • Failure of HR to rise with increased exercise intensity.

  • Noticeable change in heart rhythm by palpation, auscultation, or ECG.

  • Request to stop by the client.

  • Physical or verbal manifestations of severe fatigue.

  • Failure of testing equipment.

YMCA Cycle Ergometer Test

  • Equipment: Mechanically or electrically braked cycle ergometer, stopwatch, HR and BP monitors, RPE scale.

  • Procedures:- Each stage lasts for 3 minutes.
    - Take HR in the last 10 seconds of the 2nd and 3rd minutes of each stage.
    - If the difference in HR is < 5 bpm, a steady state is assumed and you may start another stage.
    - If not, then add another minute to the current stage.
    - Rider must maintain 50 RPM at all times – if not the test ends.
    - Terminate the test when the client reaches 85% of age-predicted HRmax or if termination signs appear.

Estimating VO2max from YMCA Test

  1. Plot average HR on Y-axis and corresponding WR on the x-axis.

  2. Construct a trend line until age-predicted HRmax.

  3. Identify the x-axis corresponding value to estimate WRmax.

  4. Use the equation to predict VO2max:

Astrand-Rhyming Cycle Test

  • Equipment: Mechanically or electrically braked cycle ergometer, HR monitor, stopwatch.

  • Procedure: - Cadence at 50 rpm for a 6-min constant WR test.
    - Set work-rate:
    - Males, unconditioned = 300 or 600 kg/m/min
    - Males, conditioned = 600 or 900 kg/m/min
    - Females, unconditioned = 300 or 450 kg/m/min
    - Females, conditioned = 450 or 600 kg/m/min
    - Take HR after 2 min.
    - If HR > 120 bpm = continue at WR for the 6 minutes.
    - If HR < 120 bpm = ↑ WR until HR > 120 after 2 min.
    - Take the average of HR at the end of the 5th and 6th minutes.

Example: Astrand-Rhyming Cycle Ergometer Test

  • Client Data: - Age: 57-year-old female, weight: 145 pounds (65.8 kg).
    - Work rate = 600 kg.m.min-1.
    - HR after second minute = 122 beats/min.

    - Convert any remaining seconds into minutes (e.g., 12:30, divide 30 by 60 to get 0.5, resulting in total time of 12.5 minutes).

    - Measure HR immediately after.
    - Estimate VO2max:

        3. Place the left hand, palm facing outward, along the spine on the lower back. 
        4. Attempt to overlap fingertips as far as possible. 
        5. Measure fingertips overlap as positive values or gap as negative values. 
        6. Repeat the procedure on each side.
