Core Definitions & Key Terms
Tissue Integrity: Integumentary, mucous membrane, corneal, or subcutaneous tissues uninterrupted by wounds.
Impaired Tissue Integrity: Damage to integumentary, mucous membrane, corneal, or subcutaneous tissues.
Impaired Skin Integrity: Defined specifically as alterations to the dermis and epidermis.
Allergic Contact Dermatitis: A cell-mediated (Type IV) hypersensitivity response triggered when skin contacts an allergen.
Alopecia: Loss of hair.
Melena: Dark sticky feces containing partly digested blood.
Urticaria: Vascular reaction of the skin characterized by erythema and wheals (hives).
Normal Physiology of the Integumentary System
Integumentary Components & Functions
The integumentary system includes the skin, hair, nails, and sweat, sebaceous, and mammary glands.
The skin is the largest organ in the human body.
Protection: Guards underlying tissues from microorganisms, chemical irritation, and mechanical trauma.
Sensation: Sensory nerves facilitate awareness of touch, pain, pressure, heat, and cold.
Thermoregulation: Regulated by the central and autonomic nervous systems via blood vessel dilation/constriction and sweat secretion by eccrine glands.
Excretion: Sweat glands eliminate waste solutions containing water, electrolytes, and urea.
Vitamin Synthesis: Synthesizes Vitamin D from sunlight/ultraviolet light exposure.
Epidermal Layers
The epidermis is composed of epithelial cells and has 5 layers on the palms and soles, and 4 layers over the rest of the body.
Stratum Corneum: Outermost layer; comprises approximately of total epidermal thickness. Contains keratin, a fibrous water-repellent protein providing tough protection.
Stratum Lucidum: Lies directly below the stratum corneum only in areas of thick skin (palms and soles).
Stratum Granulosum: Layer cells thick; contains glycolipids that slow transepidermal water loss. Keratinization (thickening of plasma membrane) begins here.
Stratum Spinosum: Several cells thick; contains abundant cells arising from bone marrow that migrate to the skin.
Stratum Basale: Deepest layer; contains keratinocytes (produce keratin) and melanocytes (produce melanin pigment to shield keratinocytes and dermal nerve endings from UV damage).
Dermal and Subcutaneous Structure
Dermis: Flexible connective tissue layer richly supplied with blood vessels, nerve fibers, and lymphatic vessels. Contains most hair follicles, sebaceous glands, and sweat glands.
Papillary Layer: Contains microscopic ridges indenting the epidermis, capillaries, and receptors for touch and pain.
Reticular Layer: Deeper layer containing blood vessels, glands, deep pressure receptors, and dense bundles of collagen fibers.
Subcutaneous Tissue (Hypodermis): Located beneath the dermis; composed of loose connective tissue and stores roughly of total body fat. Serves as thermal insulation, mechanical cushioning, and energy storage.
Cultural Considerations, Skin Color Variations, and Diversity
Biological Variations in Skin Pigmentation
Individuals with lighter skin produce less melanin; individuals with darker skin produce more melanin.
Darkest skin tones historically align with regions near the equator with intense UV exposure (Pusirid et al., 2023).
In darker skin, melanocytes may exhibit an exaggerated response to tissue injury, causing hyperpigmentation at healed lesion sites (Markovich et al., 2022).
Melasma: Overproduction of melanin causing discolored facial patches (cheeks, upper lip, chin, forehead) after sun exposure. Bleaching creams or coverup cosmetics can cause irritation or interact with topical medications (Ludman, 2022).
Assessment Techniques for Darker Skin Tones
Oxygenation & Color Changes: Inspect least pigmented areas: buccal mucosa, lips, tongue, nail beds, palms of hands, and soles of feet (Pusirid et al., 2023).
Pallor: Presents as an ashen gray color or a yellowish-brown tinge.
Cyanosis: Most prominent in nail beds, lips, and oral mucosa.
Jaundice: Inspect palms and soles. Do not confuse normal subconjunctival yellow fatty deposits in dark sclerae with true jaundice.
Pressure Injury Assessment: Darker skin rarely exhibits an identifiable blanching response. Apply light pressure to check for areas darker than surrounding skin, or skin that feels taut, shiny, or indurated (thick/firm). Assess local temperature and edema. Moistening the skin with tap water can highlight underlying color variations (Black et al., 2023).

Color Awareness: Health assessment practice recognizing skin color relevance to clinical presentation across ethnicities.
Post-Inflammatory Hyperpigmentation: Common in African American individuals; inflammatory processes alter melanin synthesis/release following injury or treatment with electromagnetic devices like ultrasound (Schwartz, 2023).
Vitiligo: Occurs at disproportionate rates in African Americans; characterized by loss of melanocytes creating depigmented blotches (Fenckel and Prunnel, 2024).
Cultural Home Remedies: Patients may use remedies like olive oil, honey, or lemon juice; nurses must explore regimens without judgment.
Alterations to Tissue Integrity and Skin Disorders Classification
Safety Alert: Latex Allergy
Estimated of Health Care Providers (HCPs) are allergic to latex (Nguyen et al., 2023; OSHA).
Attribute to widespread latex glove usage and increased reporting.
Contact dermatitis to latex is a Type IV T-cell-mediated delayed reaction.
Type I anaphylactic reactions begin within minutes and are severe/life-threatening.
Patients with latex allergies require clear identification and a latex-free environment; severely affected HCPs may require job relocation.
Systemic Classification of Skin Disorders
Infectious: Caused by bacterial, fungal, viral, or parasitic agents (e.g., impetigo, athlete's foot, chickenpox, lice). Fungal infections are particularly destructive in immunocompromised hosts. Human papillomavirus (HPV) increases genital/GI tract cancer risks (CDC, 2024A; MedlinePlus, 2024).
Inflammatory: Associated with glandular overactivity, hormone surges, or environmental exposure (e.g., acne, burns, eczema, dermatitis, psoriasis, sunburn).
Neoplastic: Skin cancers stemming from cell damage (e.g., squamous cell carcinoma, basal cell carcinoma, malignant melanoma). Melanoma is the most dangerous neoplasm (US Cancer Statistics Working Group, 2024).
Contact Dermatitis and Inflammatory Conditions
Contact Dermatitis Pathophysiology
Inflammatory skin condition characterized by damage to dermis and epidermis, presenting as a pruritic red rash. May form bullae, vesicles, or wheals corresponding to contact exposure.
Allergic Contact Dermatitis: Immune-mediated cell-hypersensitivity response to an allergen. Delayed onset. Older adults are more susceptible to allergic contact dermatitis due to lifetime cumulative exposure and sensitization (Johnson and Anjou, 2023; Ruenger, 2023b).
Irritant Contact Dermatitis: Non-immunologic direct chemical/physical damage to skin (acids, soaps, detergents, dyes, metals, perfumes). Rapid or cumulative onset. Highest incidence occurs in infants; older adults have diminished irritant reactions due to reduced dermal circulation and slow stratum corneum turnover (Fronway and Reiter, 2023).
Primary and Secondary Skin Lesions
Primary Skin Lesions (Arise from previously intact, healthy skin):
Macule: Flat, nonpalpable skin color change ().
Patch: Flat, nonpalpable skin color change ().
Papule: Elevated, solid palpable mass ().
Nodule: Elevated, solid palpable mass extending deeper into dermis ().
Tumor: Elevated solid mass ().

Lichenification: Rough, thickened area of epidermis secondary to chronic rubbing/scratching.
Scar: Connective tissue repair following destructive lesion.
Keloid: Hypertrophic scar tissue extending beyond original wound boundaries.
Excoriation: Linear erosion of epidermis caused by scratching or abrasion.
Fissure: Linear crack or split extending into dermis.
Erosion: Loss of superficial epidermal layer; non-bleeding.
Ulcer: Deep loss of skin tissue extending into dermis or deeper structures.
Classification and Emergency Management of Wounds
Wound Etiology and Depth
Intentional: Created for therapeutic purposes (e.g., surgical incisions, venipuncture).
Unintentional: Accidental trauma (e.g., laceration from motor vehicle accident, compound fracture).
Closed Wound: No rupture of skin surface despite underlying tissue trauma (e.g., contusion).
Open Wound: Destruction of skin or mucous membrane barrier.
Partial Thickness Depth: Involves epidermis and dermis; heals via epithelial regeneration.
Full Thickness Depth: Involves epidermis, dermis, subcutaneous tissue, and potentially underlying muscle/bone; requires complex connective tissue repair.
Contamination Categories
Clean: Uninfected, minimal inflammation; respiratory, alimentary, genital, and urinary tracts are not entered.
Clean-Contaminated: Surgical wound entering respiratory, alimentary, genital, or urinary tract under controlled conditions without evidence of infection.
Contaminated: Open, fresh, accidental wounds; surgical sites with major sterile technique breaks or massive GI contents spillage; acute non-purulent inflammation present.
Dirty or Infected: Purulent drainage or dead tissue present; clear clinical infection.
Emergency First-Aid Guidelines for Untreated Wounds
Control Severe Bleeding: 1) Apply direct pressure directly over wound site; 2) Elevate the involved extremity above heart level.
Prevent Infection: 1) Flush/clean abrasions and lacerations using normal saline; 2) Cover wound with clean or sterile dressing. Wrap firmly to approximate edges and maintain pressure. If blood saturates the initial dressing, apply a second dressing directly over top—never remove the first dressing layer to prevent disturbing blood clots.
Reduce Pain & Swelling: Apply ice packs to wound and surrounding area.
Monitor Shock: Evaluate for rapid thready pulse, cold/clammy skin, pallor, and reduced blood pressure.

Newborn Tissue Integrity
Vernix Caseosa: Waxy layer of lipids, shed cells, and sebum protecting fetal skin in utero; provides anti-infective, thermoregulatory, moisturizing, and antioxidant qualities (Bam Allen et al., 2023; Kaneshiro, 2023).
Common Benign Newborn Findings: Milia, neonatal acne (maternal hormone surge), Erythema Toxicum (blotchy rash appearing post-delivery, clearing by ), Stork Bites (telangiectatic nevi fading by ), Mongolian spots, cafe-au-lait spots, port-wine stains, hemangiomas.
Anatomical Differences: Newborn skin is thinner than adult skin with sparse subcutaneous fat, causing accelerated heat loss, poor temperature regulation, and high chemical/topical drug absorption (Ball et al., 2022).
Pediatric Tissue Integrity
High Body Surface Area (BSA) to mass ratio elevates risk for thermal loss, fluid dehydration, and topical toxin absorption. Skin progressively thickens into the (Stomidas et al., 2023).
Erythema Infectiosum (Fifth Disease): Parvovirus B19 (PVB19). Slapped-cheek facial rash with lacy exanthem on body. Self-limiting (); spring/early summer. Treated with rest and fluids (Kostolansky and Weymak, 2023).
Pityriasis Rosea: Viral-origin rash. Begins with a solitary "herald patch" on trunk, followed by pink/tan scaly eruptions along skin cleavage lines. Duration .
Roseola Infantum: Human Herpesvirus 6 (HHV-6). Occurs in children . High fever for followed by rapid rash emergence upon fever defervescence. Transmitted via respiratory droplets (incubation ). Complication: febrile seizures (Johns Hopkins Medicine, 2024D; Mullens and Krishnamurti, 2023).
Adolescents
Testosterone increases sebum production and axillary apocrine sweating.
Female menstrual cycles induce premenstrual exacerbations of eczema, psoriasis, and systemic lupus erythematosus.

Pregnancy
Hormonal skin manifestations: Hyperpigmentation (linea nigra, chloasma), striae gravidarum (stretch marks), pruritus gravidarum.
Atopic Eruption of Pregnancy (AEP): Occurs in of pregnancy-related skin disorders. Eczematous lesions on neck and limb flexor surfaces prior to third trimester. Resolves postpartum; fetus unaffected, but child has increased atopic dermatitis risk (Curian et al., 2024; Pomerantz, 2023).
Older Adults (Table 21.6 Skin Aging)
Intrinsic Aging: Begins ~age (exfoliation slows). By age , moisture transfer dermis-to-epidermis declines and fat cells shrink. By age , collagen synthesis stops. Sebaceous glands shrink, causing skin dryness and fragility. Menopause drops estrogen, thinning skin further.
Extrinsic Aging: Environmental UV exposure and free radical damage cause cornified layer thickening, solar elastosis, deep wrinkles, precancerous actinic keratoses, and freckling (Kruppmann et al., 2021).
Functional Changes: Subcutaneous fat loss/redistribution (loss on face/hands, gain on abdomen/thighs) impairs thermoregulation and cushioning over bony prominences. Melanocytes decline (paler skin), but localized solar lentigines (age spots) form. Dermal microvessel fragility causes senile purpura (Bradke, 2022).
