Overview of Cranial Nerves

  • A total of twelve cranial nerves are present in humans, each designated with a Roman numeral (I-XII). These nerves have various functions, primarily related to sensation and motor control of the head and neck region.

Cranial Nerve List

  • I: Olfactory n.
  • II: Optic n.
  • III: Oculomotor n.
  • IV: Trochlear n.
  • V: Trigeminal n.
  • VI: Abducent n.
  • VII: Facial n.
  • VIII: Vestibulocochlear n.
  • IX: Glossopharyngeal n.
  • X: Vagus n.
  • XI: Accessory n.
  • XII: Hypoglossal n.

Cranial Nerve VII – Facial Nerve

Overview

  • The facial nerve arises from the ponto-medullary junction and is primarily responsible for the muscles of facial expression, taste sensation, and certain secretory functions.
  • It can be divided into two roots:
    • Nervus Intermedius: Contains sensory and autonomic fibers (General Somatic Afferent (GSA), Special Visceral Afferent (SVA), General Visceral Efferent (GVE), and a small amount of General Visceral Afferent (GVA)).
    • Motor Root: Contains Special Visceral Efferent (SVE) fibers.

Pathway

Intracranial Pathway
  1. Originates from the pontomedullary junction.
  2. Passes through the internal acoustic meatus.
  3. Travels through the facial canal and gives off three branches within the petrous portion of the temporal bone.
  4. Exits via the stylomastoid foramen.
Extracranial Pathway
  1. Leaves the stylomastoid foramen.
  2. Ascends into the parotid gland.
  3. Divides into five branches to innervate muscles of facial expression: temporal, zygomatic, buccal, mandibular, and cervical branches.

Facial Nerve Fibers and Functions

  • General Somatic Afferent: Supplies a small area of the external ear.
  • Special Visceral Efferent (SVE):
    • Muscles of facial expression.
    • Posterior belly of digastric muscle.
    • Stylohyoid muscle.
    • Stapedius muscle.
  • Special Visceral Afferent (SVA): Taste from the anterior two-thirds of the tongue and soft palate.
  • General Visceral Efferent (GVE): Parasympathetics to sublingual, submandibular, and lacrimal glands.
  • General Visceral Afferent (GVA): Mucosa of the nasal cavity and soft palate.

Branches of the Facial Nerve

1. Greater Petrosal Nerve
  • Arises from the geniculate ganglion and carries:
    • GVE: Pre-ganglionic parasympathetic fibers.
    • SVA: Taste fibers from the soft palate.
    • GVA: Sensory fibers from the soft palate and nasal cavity.
  • Leaves petrous temporal bone via the hiatus of the canal for greater petrosal nerve and enters the pterygoid canal.
2. Nerve to Stapedius
  • Carries Special Visceral Efferent fibers to the stapedius muscle which dampens sound to protect structures of the inner ear.
3. Chorda Tympani Nerve
  • Travels from the middle ear cavity through the petrotympanic fissure to join the lingual nerve, carrying:
    • SVA: Taste fibers from the anterior two-thirds of the tongue.
    • GVE: Pre-ganglionic parasympathetic fibers to the submandibular ganglion.

Cranial Nerve VIII – Vestibulocochlear Nerve

Overview

  • The vestibulocochlear nerve is responsible for hearing and balance, comprising two branches:
    • Cochlear Branch: Involved in the sense of hearing.
    • Vestibular Branch: Involved in balance and spatial orientation.

Cochlear Branch

  • Sound waves are perceived by hair cells within the spiral organ of Corti.
  • Hair cells transduce sound waves into electrical signals (auditory transduction).
  • Cochlear nerve carries special somatic afferent fibers and exits the temporal bone via the internal acoustic meatus.

Vestibular Branch

  • Senses head movements (rotation, acceleration, and deceleration) via hair cells located in the macula of the utricle/saccule and crista ampullaris of semicircular ducts.
  • Hair cells transduce vestibular stimuli into electrical signals (vestibular transduction).
  • Vestibular nerve carries special somatic afferent fibers and exits the temporal bone via the internal acoustic meatus.

Spiral Ganglia

  • Located at the modiolus of the cochlea.
  • Contains cell bodies of bipolar neurons involved in hearing.
  • Peripheral processes contact hair cells, while central processes connect to the brainstem.

Vestibular Ganglion

  • Located at the distal end of the internal acoustic meatus.
  • Contains cell bodies of first-order bipolar vestibular neurons.

Cranial Nerve IX – Glossopharyngeal Nerve

Overview

  • The glossopharyngeal nerve primarily serves the posterior third of the tongue, pharynx, and various autonomic functions.

Fiber Types and Functions

  • General Somatic Afferent: Supplies posterior one-third of the tongue and medial side of the tympanic membrane.
  • Special Visceral Efferent: Innervates stylopharyngeus muscle (derived from the third pharyngeal arch).
  • Special Visceral Afferent (SVA): Taste sensation from the posterior one-third of the tongue.
  • General Visceral Efferent (GVE): Parasympathetic innervation to the parotid gland.
  • General Visceral Afferent (GVA): Sensory fibers from the palatine tonsils, mucosa of the lower nasopharynx and oropharynx, carotid body, and sinus.

General Pathway

  • Originates from the anterolateral surface of the upper medulla oblongata.
  • Exits the skull via the jugular foramen and gives off several important branches, including:
    • Tympanic nerve.
    • Nerve to stylopharyngeus muscle.
    • Carotid nerve.
    • Three terminal sensory branches (pharyngeal, tonsillar, and lingual).

Autonomic Function

Carotid Body and Sinus
  • Carotid Body: Contains chemoreceptors that sense blood O2, CO2, and pH levels;
    • Modulates respiratory rate and cardiac output.
  • Carotid Sinus: Contains baroreceptors that detect blood pressure changes.

Clinical Note

  • Gag Reflex:
    • Protective response involving contraction of the throat when stimulated.
    • Sensory component: CN IX (Glossopharyngeal).
    • Motor component: CN X (Vagus).

Cranial Nerve X – Vagus Nerve

Overview

  • The vagus nerve plays a critical role in visceral function, regulating heart rate, respiratory rate, and digestive processes by providing parasympathetic innervation to thoracic and abdominal organs.

Fiber Types and Functions

  • General Somatic Afferents: Sensation from parts of the external acoustic meatus.
  • Special Visceral Efferents: Muscles of the pharynx, larynx, and soft palate (with exceptions).
  • General Visceral Efferents: Parasympathetic fibers to thoracic and abdominal organs.
  • General Visceral Afferents: Mucosa of the laryngopharynx and larynx, organs within the foregut and midgut, chemoreceptors in the aortic body, and baroreceptors in the aortic arch.

Pathway

  • Leaves the anterolateral surface of the medulla oblongata; exits via the jugular foramen alongside CN IX and XI.
  • Contains two ganglia:
    • Superior (jugular) ganglion: smaller and located within jugular foramen; sensory cell bodies.
    • Inferior (nodose) ganglion: larger and located below jugular foramen; sensory cell bodies.

Branches of the Vagus Nerve

  • Includes the auricular nerve, pharyngeal branches, superior and recurrent laryngeal nerves, and contributions to various plexuses (e.g., cardiac, pulmonary, esophageal).

Reflex Functions

  • Auricular Branch:
    • Provides sensation from the rear of the external acoustic meatus; stimulation can cause reflexive coughing, vomiting, or in rare cases, cardiac arrest.

Recurrent Laryngeal Nerve

  • Provides motor innervation to all intrinsic muscles of the larynx except cricothyroid; sensory to mucosa below vocal folds.

Cranial Nerve XI – Accessory Nerve

Overview

  • Divided into the spinal root and cranial root.
Spinal Root
  • Originates from the upper five cervical spinal cord levels; travels with the vagus nerve to innervate the sternocleidomastoid and trapezius muscles.
Cranial Root
  • Arises from the lateral portion of the medulla; often absent in a majority of cases.

Cranial Nerve XII – Hypoglossal Nerve

Overview

  • Responsible for motor control of the tongue’s intrinsic and extrinsic muscles.

Pathway

  • Originates from the medulla oblongata and exits via the hypoglossal canal; travels with the ventral root of C1.
