Overview of Cortical Syndromes

  • Definition: Cortical syndromes refer to a constellation of symptoms arising from dysfunctions of the cortex with some exceptions noted.

  • Key Roles of the Cortex:

    • Outer layer of brain tissue

    • Contains grey matter and cell bodies of neurons

    • Plays critical roles in attention, awareness, thought, language, and more.

Hemispheric Functions

Dominant vs. Non-Dominant Hemispheres
  • General Information:

    • The cortex functions may vary between the dominant (typically left) and non-dominant (typically right) hemispheres.

    • Dominant hemisphere is primarily responsible for language functions.

  • Statistical Insights:

    • Approximately 95% of right-handed individuals show left hemisphere dominance for language.

    • About 30-40% of left-handed individuals have a right hemisphere dominance for language.

Dominant Hemisphere (Primarily Left)
  • Functions:

    • Language processing (Broca's and Wernicke's areas)

    • Complex motor planning (praxis)

    • Sequential and analytical arithmetic skills

    • Musical ability related to logical structure

    • Sense of direction: Following set paths

Non-Dominant Hemisphere (Primarily Right)
  • Functions:

    • Prosody (emotional content of speech)

    • Visuospatial analysis and spatial attention

    • Estimations of quantity and mathematical tasks like lining up numbers

    • Musical ability relating to emotional aspects

Language and Its Anatomy

Circuitry of Language
  • Key Areas:

    • Broca's Area:

    • Responsible for speech fluency.

    • Located near the motor cortex responsible for facial movements.

    • Wernicke's Area:

    • Involved in language comprehension.

    • Projects to the primary auditory cortex.

  • Connections:

    • Arcuate fasciculus: The white matter pathway connecting Broca's and Wernicke's areas.

Language Processing Steps
  1. Input: Auditory information reaches the primary auditory cortex.

  2. Comprehension: It is processed in Wernicke’s area for meaning recognition.

  3. Articulation: Subsequently, motor cortex (Broca’s area) coordinates speech production.

Testing Language at Bedside

  • Methods:

    1. Spontaneous speech regarding fluency, prosody, grammar, and articulation.

    2. Naming ability assessment.

    3. Comprehension tasks.

    4. Repetition of phrases.

    5. Reading and writing capabilities.

Localization of Lesions in Aphasias

  • Aphasias are common language impairments related to specific brain lesions.

  • Broca’s Aphasia (dominant left frontal lobe):

    • Symptoms: Impaired naming, repetition, and non-fluent speech. Comprehension largely intact. Patients experience frustration due to awareness of the deficit.

  • Wernicke’s Aphasia (dominant left temporoparietal):

    • Symptoms: Impaired comprehension, meaningful speech but nonsensical output. Patients often unaware of their condition.

  • Global Aphasia: Impairment in fluency, comprehension, and repetition, often seen with strokes affecting the MCA.

Transcortical Aphasias

  • Types:

    • Transcortical Motor Aphasia: Similar to Broca’s aphasia, but with preserved repetition.

    • Transcortical Sensory Aphasia: Similar to Wernicke’s aphasia, but also with intact repetition.

    • Mixed Transcortical Aphasia: Both expressive and receptive language are impaired, but repetition skills remain.

Other Syndromes Related to Aphasia

Alexia and Agraphia
  • Alexia: Impairment in reading due to lesions in the dominant occipital cortex.

  • Agraphia: Impairment in writing, often seen with aphasia related to the inferior parietal lobule.

    • Writing may be agrammatical and sparse, especially with Broca’s aphasia.

  • Gerstmann's Syndrome: Agraphia, acalculia, right-left disorientation, and finger agnosia associated with angular gyrus lesions.

  • Color Anomia: Common in patients with Alexia without agraphia due to occipital cortex lesions.

Audio and Speech Production Disorders Mistaken for Aphasia

  • Dysarthria and Aphemia:

    • Dysarthria: Disorder of motor production of speech.

    • Aphemia: Verbal apraxia without language impairment; difficulty with speech articulation may occur.

  • Auditory Disorders:

    • Peripheral hearing loss and pure word deafness related to dominant auditory cortex lesions.

    • Cortical deafness with bilateral lesions results in an inability to decipher words.

  • Psychiatric Disorders:

    • Conditions such as schizophrenia can manifest with symptoms similar to aphasia.

Apraxia

  • Definition: The inability to perform complex purposeful movements that should be ingrained in muscle memory.

  • Examples: Tasks like brushing teeth or hair.

  • Verbal Apraxia (Aphemia): Difficulty specifically associated with speech production despite intact comprehension.

Neglect Disorders

Hemineglect
  • Definition: A failure to attend to stimuli on one side of the body, often associated with right parietal cortex lesions.

  • Testing Neglect: Patients may be asked to identify objects in complex visual scenes or to place numbers on a clock.

Hemispatial Neglect and Variants
  • Localization: Typically, the nondominant hemisphere (right) processes visual attention and spatial awareness.

  • Neglect Syndromes include visual neglect or extinction and anosognosia.

Spatial Processing Disorders

  • Disorders: Associated primarily with the parietal cortex affecting attention and visual processing in space.

Visual Processing Disorders

Cortical Blindness (Anton’s Syndrome)
  • Characteristics:

    • Complete vision loss without awareness of it.

    • Caused by lesions to bilateral primary visual cortex.

Balint Syndrome
  • Clinical Features:

    • Simultanagnosia: Difficulty perceiving the visual scene holistically.

    • Optic ataxia and ocular apraxia.

Lesions to Inferior Occipitotemporal Cortex
  • Prosopagnosia: Inability to recognize familiar faces; involves the bilateral fusiform gyri.

  • Achromatopsia: Inability to recognize colors due to lesions in the contralateral inferior occipitotemporal cortex.

Frontal Lobe Functions

  • Importance: The frontal lobe is crucial for personality traits, organization, and judgment.

Forms of Frontal Lobe Dysfunction
  1. Dorsolateral Lesions: Result in executive dysfunction and impaired working memory.

  2. Medial Lesions: Lead to emotional changes, loss of initiative, and apathy.

  3. Orbitofrontal Lesions: Cause impulsive behavior and poor judgment.

Differential Diagnosis of Frontal Lobe Disorders

  • Commonly affected by a variety of causes including:

    • Head trauma, infarcts (ACA or MCA), hemorrhage, tumors, infections, and degenerative disorders such as Alzheimer's disease.

Neuropsychological Testing

  • Purpose: Evaluates cognitive function in detail, often requiring extensive time beyond typical clinical visits.

  • Uses: Especially relevant in cases of dementia, epilepsy, and other cognitive impairments.

Common Tools Used in Testing
  • Mini Mental Status Exam: Most commonly employed.
