• Absent policy
• Only one valid & excused absence allowed across seven sessions
• Medical certificate required within 1-2 days for health-related absence – otherwise fail
Internship Time Frame & Grading
• Official counting of hours begins ONLY after IELAP issues deployment memo – earliest 04/15/2025
• Final submission of all OneDrive requirements: 07/18/2025 11 : 59 PM
• Evaluation of folders: July 2025; grade encoding on/before 07/24/2025
• Grading scale
• = PASS
• = INC (incomplete)
• = FAIL
• INC blocks on-time graduation; completion processing starts 07/25/2025 onward
Internship Hours & Work-Setup Rules
• Ideal duty: Mon–Fri 8 AM-5 PM (face-to-face, hybrid or WFH)
• Graveyard / shifting allowed if approved & waiver signed
• Overtime limits
• Daily: max h (twice/week); other days h guideline
• Weekly: max h (Mon-Fri)
• Saturday OT allowed only when NO Saturday class; Sat hours are on top of weekly cap but must be documented

• Inside root you must see exactly four folders

Pre-Internship Requirements (Upload BEFORE deployment – 6 files)
• Briefcase résumé (if portal down, use “Student Resume” template from Canvas)
• Signed Student Consent Form (2 pp; parent/guardian ID attached; face-to-face & hybrid interns only)
• Health Services Unit (HSU) certificate
• Tests: chest X-ray, urinalysis, fecalysis, CBC w/ platelet, physical & neuro exam
• Validate at HSU clinic then scan
• Screenshot of COR (3rd Term SY 24-25, status = ENROLLED)
• Signed Tracking Form (interview proof; include date, mode & company signature)
• Screenshot of Tracking Form Survey submission receipt (campus-specific link)
Mid-Internship Requirements (Upload @ 6 weeks – 3 files)
• Notarized Training Agreement signed by

• MIPEF – Mid Internship Performance Evaluation Form
• Criteria: Work & Professional Values 15 %, Work Efficiency 15 %, Communication 15 %, Interpersonal 15 %, Technical 40 %
• All fields incl. Overall Rating and Narrative must be filled; no blanks (use “NA” if not applicable)

Post-Internship Requirements (Upload 2 wks BEFORE end – 8+ files)
• Internship Monitoring Form (IMF – Excel)
• Update daily; use hour-forecast sheet to predict completion date
• Signed Post Internship Performance Evaluation Form (PIPEF) + screenshot of its M-Form
• Internship Project Report (IPR)
• Use Canvas template; turn all red texts to black; Section VI (a-c) = 5 sentences each; signed by supervisor
• Internship Program Evaluation (IPE) PDF + M-Form screenshot
• Certificate of Completion (company header, hours, exact dates, “Given this __ day…” + signature)
• Supervisor & evaluator IDs (gallery in Post folder)
• CS / IT ONLY: Completed Rubrics PDF + M-Form screenshot

• Use one continuous thread per topic (OT, leave, submissions, waivers)
• Proper scanning
• Use CamScanner / phone scanner; remove staples, shadows; ensure clear text before upload
Sick-Leave / Absence Flow
• Email at least 2-3 days before planned leave (half- or full-day) with table:

Overtime (OT) Flow
• Planned OT – email prior; include:

Roles & Responsibilities
Industry Partner (IP)
• Comply with labor & H&S laws; provide course-related tasks; assign supervisor; evaluate intern
• No displacement of employees; no hazardous work; no Saturday duty during class
• Inform IELAP of daybreak shifts, field duty, out-of-town trips, special projects
Student Intern
• Must be ≥ y/o; attend orientation; submit medical & COR
• Observe dress code, punctuality, professionalism; report problems to supervisor & adviser
• Maintain confidentiality; secure waivers for OT/field/graveyard
• Responsible for any damage caused by negligent acts; follow health & safety protocols
FEU / IELAP
• Provide orientation, site visits, insurance, data-privacy-compliant data sharing
Professional & Ethical Conduct
• Punctuality & attendance – respect schedules
• Professional communication – clarity, trust, documentation
• Collaboration & networking – build relationships; seek feedback for growth
• Respect boundaries; uphold FEU’s reputation – “You are the school’s ambassadors”
Internship Behaviours That Trigger Failure
• Starting duty before IELAP deployment memo
• >1 unexcused/invalid Saturday absence
• Missing document deadlines (auto INC then possible FAIL)
• Email & OneDrive not following prescribed formats

• QR-code / link for Internship Saturday class – use SAME room whole term
• CAMScanner (Android/iOS) recommended for document digitisation
Remember: Final all-in submission via OneDrive on or before 07/18/2025 23:59. Keep a personal backup of every file and e-mail acknowledgment.
