  • Treatment prescribed:

    • Physical therapy.
  • Initial Evaluation:
    • Conducted 1 week post-injury.
    • Patient expresses a desire to return to power walking.
  • Past Medical History (PMH): Reported osteoporosis for 1 year.
  • Additional Findings: Intake form reveals episodes of leaking urine within the last 3 months.

Change in Status

    • Therapy Completion: Attended 7 out of 10 visits (3.5 weeks total).
    • Missed Visits: Did not show for the 8th or 9th visits due to hospitalization.
  • Hospitalization Causes:

    • Daughter took her to the ER due to these changes.
    • Diagnosis during hospitalization: Urinary tract infection and dehydration.
