Evaluation Process

  • Terminology

    • Screening: is OT eval needed? What assessments do you think may be helpful if evaluating (brief meaOsure)

    • Assessment: a tool to guide eval

    • Eval: rundown of areas that need to be worked on

  • Purpose of eval: baseline, to better tailor OT to needs, insurance companies like to see what they are paying for

  • When do evals occur: after OT need is established (initial), then do reeval or discharge eval

  • Process of Eval

    • Occupational Profile

      • Hx, intraprofessional team, interview (child/caregiver)

    • Occupational analysis

      • Methods of eval (what assessments), how & where is eval, supports and barriers, all domains of practice

    • Interpation

      • client values, strengths and needs, support/context, determine outcomes, create goals, select outcome measures/procedures

    • Interpersonal skills

      • rapport building

      • client-centered/family-centered

      • interview skills

      • sensitivity/responsiveness

      • avoid jargon

      • honesty/transparency

      • non-verbal skills

  • Occupational anysis

    • top down: client performing whole tasks then looking DOWN to specific areas

    • Bottom up: specific areas UP to whole tasks

      • for documentation, not as an occupation-based

      • not observing skill in a natural context (could be an activity, not an occupation)

      • Not as occupation-based

    • Will effect interventions you are doing

    • In context skills, if possible

    • standized vs non standized

  • Standardized Assessments

    • fixed # of items

    • test manual: purpose, population, reliability/validity info

    • Uniform guidelines for administration/scoring

    • advantages/disadvantages

      • results same on same child (no mater who does)

      • insurance likes

      • takes time

      • costs money

      • may not be done on that pop

    • norm references (average for age group) or criterion-based (must receive a certain score to be within cut off, not comapred to same age peers, based on a list of skills)

    • Reliablity: Consistenacy of score

      • test-retest (stability of test overtime)

      • Interater (can u get same score from kid when scoring simultaneously)

    • Validity: does assement measure what it says it does

      • construct validity: the assesment mesures the theoritical construct

      • content validity: the items and accurate sample of the doamain tested

      • criterion-related validity: the assessment predicts how and individual performs on other measurements or activities

  • Non-Standarized Assessments

    • pros and cons

      • No script, maybe not even manual

      • no scores against norm

      • shorter

      • more flexible

      • insurance doesn’t love

      • results less clear

  • Chronolgical age

    • HOW TO CALCULATE

  • Process of eval

    • scoring accuratly (Scoring how tool is ment to be scored)

    • Synthesis/Interprating results

      • Thearputic reasoning (art & Sci)

  • Recomendations

    • Think of results as well as important to child & childs cargiver

    • is it important to kids life

    • What type of service delvery do they need (at home, individual, group, how much, at outpatent)

  • Documentation

    • Eval

    • Daily note

    • Progress reports

    • Reevals

    • Discharge

  • Document

    • Why OT is needed

    • What happened

    • change in ability

    • plan for next session

    • WHY IS IT OCCUPATION-BASED

    • Input from kids and fam

    • objective data

  • Communication regarding report

    • Talk to the intraprofessional team

    • talk to parents

    • might also be telling parents in person

    • Parents might have an emotional response
