Assessment Objectives
Global Criteria
- Explain assessment results to the client
- Communicate findings clearly to enhance client understanding and involvement.
- Identify and rank the problems needing resolution
- Collaborative effort to pinpoint crucial issues to address in treatment.
Goal Setting
Global Criteria
- Formulate agreed-upon immediate and long-term goals
- Establish clear and achievable objectives for the client’s treatment journey.
- Identify treatment methods and resources to be utilized
- Select appropriate interventions tailored to the client's needs.
Treatment Plan Requirements
- Basis of the client’s treatment plan is the assessment
- Treatment should stem from comprehensive evaluation of the client.
- JAHCO Requirements:
- Treatment plan must be based on Intake information.
- Must be formulated within 72 hours of Intake.
Individualized Treatment
- The treatment plan should be individualized
- Customized approaches based on specific client circumstances and goals.
Collaborative Problem-Solving
- Client and Counsellor list the significant problems and rank them
- Joint activity to ensure all pertinent issues are addressed and prioritized in the treatment plan.
