Introduction to Erythrocyte Indices

  • Erythrocyte indices are essential measurements used for detailed analysis of red blood cells (RBCs).
  • These indices are crucial for determining:
    • The size of red cells
    • The content of hemoglobin (Hb) in red blood cells
    • The concentration of hemoglobin in red blood cells
  • They are particularly useful in the morphologic characterization of anemias.
  • Erythrocyte indices can be calculated from:
    • Red cell count
    • Hemoglobin concentration
    • Hematocrit (Hct)

Key Erythrocyte Indices

  • The four primary erythrocyte indices include:
    • Mean Cell Volume (MCV)
    • Mean Cell Hemoglobin (MCH)
    • Mean Cell Hemoglobin Concentration (MCHC)
    • Red Cell Distribution Width (RDW)
Mean Cell Volume (MCV)
  • Definition: MCV represents the average size of red blood cells.
  • Measurement:
    • Measured directly by automated instruments from the volumetric measurement of each red cell.
    • For semi-automated instruments and manual methods, MCV can be calculated as:

  • Units: Expressed in femtoliters (fl), where 1 femtoliter = 10^-15 liters.

Peripheral Blood Smear
  • A normal peripheral smear shows erythrocytes that appear as:
    • Circular, homogeneous disks of nearly uniform size.
    • Diameter ranges from 6 to 8 μm.
    • Central pallor not exceeding one third of the cell diameter.
    • Erythrocytes are approximately the same size as the nucleus of a small lymphocyte.
Types of Red Cell Morphologies
  • Iron Deficiency Anemia:
    • Red cells may appear microcytic (smaller than normal).
    • They are also hypochromic, with decreased hemoglobin content per cell, resulting in an enlarged central pale area.
  • Macrocytic Cells:
    • Larger and thicker red cells that stain deeply.
    • Lack a central pallor,
  • Dimorphic Anemia:
    • Characterized by anisocytosis (variation in cell size) and anisochromia (variation in cell color).
    • Presence of microcytic cells, normocytic cells, and few macrocytes.
    • Common in conditions such as sideroblastic anemias and can appear after iron therapy for iron deficiency anemia or hypochromic anemia post-transfusion with normal cells.
Mean Cell Hemoglobin (MCH)
  • Definition: Average amount of hemoglobin in a single RBC.
  • Calculation:

  • Units: Expressed in picograms (pg).
  • Normal Range: 27-32 pg.

Mean Cell Hemoglobin Concentration (MCHC)
  • Definition: Concentration of hemoglobin in a given volume of packed red cells.
  • Calculation:

  • Units: Expressed in grams/dl or grams/liter.
  • Normal Range: 30-35 grams/dl.
  • Clinical Significance:
    • MCHC corresponds with the degree of hemoglobinization of red cells on a peripheral blood smear.
    • If MCHC is normal, red blood cells are normochromic.
    • If MCHC is low, cells are hypochromic.
    • MCHC can be raised in hereditary spherocytosis and decreased in hypochromic anemia.
Clinical Examples of Red Cell Characteristics
  • Hypochromic Red Cells:
    • Cells display decreased hemoglobin content resulting in larger central pale areas.
    • Associated with decreased MCH and MCHC.
  • Hereditary Spherocytosis:
    • Spherocytes are nearly round, smaller than normal red cells, and lack central pallor (hyperchromic).
    • Normal MCH, often increased MCHC due to a reduced surface/volume ratio.

Red Cell Distribution Width (RDW)
  • Definition: Quantitative measure of variability in red cell size (anisocytosis).
  • Clinical Relevance:
    • A higher RDW value indicates more heterogeneity in red blood cell sizes.
    • Normal RDW range is from 11.0 to 14.5.
    • Increased RDW is commonly observed in iron deficiency anemia, which aids in differentiating it from thalassemia trait conditions where RDW remains normal.
