Overview of the Optic Nerve

Formation of the Optic Nerve

  • The optic nerve is composed of approximately 750,000 to 1.5 million axons from ganglion cells, with a mean of 1.1 million axons.

  • The nerve fiber layer (NFL) thickens from the periphery to the optic nerve head (ONH).

  • The optic nerve exhibits retinotopic organization:

    • Axons are organized topographically within the NFL and optic nerve, with specific retinal areas corresponding to specific regions of the visual field.

Retinotopic Organization in the NFL

  • Axons from the peripheral retina run deeper in the NFL, entering at the margin of the optic disc.

  • The papillomacular bundle extends from the foveal region to the temporal rim of the optic disc.

  • Clinical Appearance: Changes in the thickness of NFL may be observed, impacting visual function.

Clinical Features of the Optic Nerve

  • Optic Nerve Head (Disc):

    • A prelaminar portion characterized as slightly vertically oval.

    • The horizontal diameter of the optic disc is typically 1.5 mm; varies with the posterior scleral foramen.

    • Center of the optic disc is approximately 4 mm medial and slightly superior to the fovea.

    • The blind spot is located about 10-15 degrees temporal and slightly below the visual field center.

Size and Appearance of the Optic Disc

  • The optic disc size and shape have clinical significance:

    • Various methods to determine disc size include:

    • Histological assessment of the posterior scleral foramen.

    • Identification of the Peripapillary Ring of Elschnig, visible as a white line at the optic disc edge.

    • Bruch's Membrane Opening (BMO): Visualized using ocular coherence tomography (OCT).

  • Neuroretinal Rim (NRR):

    • Extends from the edge of the optic disc to the outer edge of the cup, containing axons, blood vessels, and astrocytes.

    • Width varies based on the number of axons in specific regions, following the ISNT Rule (Inferior, Superior, Nasal, Temporal).

  • Cup Appearance:

    • Central excavation (if present), not color-specific, varies in size depending on the optic disc's dimensions, blood vessel quantity, and connective tissue.

    • Larger cups correspond with larger optic discs; primarily composed of astrocytes with yellow/white appearance.

ISNT Rule and Cup-to-Disc Ratio

  • ISNT Rule:

    • Describes the expected width of the NRR in the sequence of Inferior (I), Superior (S), Nasal (N), and Temporal (T).

    • Rule may not hold for very large discs.

  • Cup-to-Disc Ratio:

    • Measures the size of the cup relative to the optic disc's diameter.

    • Average C/D ratio is around 0.3, with variations across ethnic groups - often larger in individuals of African descent and smaller in women than men.

Relationships Between Disc and Cup Sizes

  • The disc size influences cup size and NRR width:

    • Genetic factors play a role in determining disc size.

    • Range measurements show:

    • Disc Size: 6.8 X Range.

    • NRR Size: 5.8 X Range.

    • Normal Expectations: Larger discs (> 4.4 mm²) typically host larger cups; while smaller discs (< 1.4 mm²) exhibit smaller cups.

Clinical Changes in C/D Ratio

  • Changes in C/D ratio can indicate disease processes, especially glaucoma.

    • A normal cup height being noted alongside pathological conditions (e.g., glaucoma showing increased cup height) suggests risk factors for optic nerve health.

Peripapillary Changes

  • Variations associated with the optic nerve:

    • Scleral Crescent: Visibility of sclera around the optic nerve due to absence of retina and choroid; often found inferior-temporally.

    • Choroidal Crescent: Presence of choroidal tissue visible around the disc edge.

    • RPE Crescent: Accumulation of RPE pigment; reflects normal anatomical variations.

    • Peripapillary Atrophy: Thinning of retinal and RPE surrounding the disc, often seen with aging, high myopia, and glaucoma.

    • Composed of two zones:

      • Zone Beta: Close to the disc edge, shows chorioretinal atrophy, loss of RPE, and large visible choroidal vessels.

      • Zone Alpha: Following Zone Beta, features irregularities in RPE pigmentation.

Anatomy of the Optic Nerve

    • Intraocular (Intrascleral): 1 mm long, approximately 1.5 mm in diameter; expands to 3 mm when leaving the eye due to myelin.

    • Intraorbital (Intraconal): 25-30 mm long and “S” shaped to accommodate eye movements.

    • Intracanilcular: Ranges from 4-10 mm and is tightly linked to optic canal walls.

    • Intracranial: Spanning 10-16 mm and runs from the optic foramen to the chiasma.
