- Asymmetric crawl or walking on toes.

- Equipment and Therapy:
- Orthotic devices and adaptive equipment.
- Functional (Neuromuscular) Electrical Stimulation.
- Intensive Occupational, Physical, and Speech Therapy.
- Surgical interventions to correct deformities or release contractures.
- Nursing Management:
- Prevention: Monitor for and prevent constipation, urinary tract infections (UTI), and skin breakdown.
- Protection: Respiratory protection and safety precautions to prevent falls and manage seizures.
- Nutrition: Nutritional supplementation as needed for feeding difficulties.
- Pharmacological Management:
- Antispasmodics: Including the use of Botulinum toxin A (Botox) and Intrathecal baclofen pumps.
- Other Medications: Analgesia for pain, Antiepileptics for seizure control, and a consistent bowel regimen.

Neural Tube Defects (NTD)
- Etiology: Occurs when the neural tube fails to close either entirely or in a small portion during embryonic development.
- Prevention through Nutrition:
- Supplementation with folic acid: doses of 0.4−0.8mg/day.
- Folic acid should be taken preconception and continued throughout pregnancy.
- Dietary sources include fortified cereal grains.
- Prenatal Diagnosis:
- Detection of elevated alpha-fetoprotein (AFP) levels in amniotic fluid.
- Routine uterine ultrasound.
- Genetic counseling to discuss birth plans and neonatal support needs.
Specific Types of Neural Tube Defects
- Anencephaly:
- Absence of both cerebral hemispheres.
- Prognosis: Many are stillborn; those born alive survive for hours to days before respiratory failure occurs.
- Care Focus: Comfort measures and hospice care.
- Spina Bifida: A general term for the failure of the bony spine to close.
- Spina Bifida Occulta: Not externally visible; typically localized at L5−S1.
- Skin markers: Dimple, port-wine angiomatous nevi, dark tufts of hair, or soft subcutaneous lipomas.
- Occult Spinal Dysraphism (Neurologic Involvement): Can involve fibrous bands, adhesions, intraspinal lipomas, or a tethered cord (where fixed structures add traction to the spinal cord).
- Manifestations: Abnormal gait, foot weakness or deformity, and disturbances in bowel or bladder sphincter control.
- Spina Bifida Cystica: A visible defect with an external saclike protrusion.
- Meningocele: The sac contains meninges and spinal fluid only; typically no neurologic deficits.
- Myelomeningocele: The sac contains meninges, spinal fluid, and nerves. This results in varying degrees of neurologic deficits. It is most common in the lumbar or lumbosacral areas. The membrane is fine, prone to CSF leakage, and easily ruptured.
- Preoperative Care:
- Intensive nursing care to prevent infection.
- Assessment of neurologic status and associated anomalies: Developmental Dysplasia of the Hip (DDH), clubfoot, or genitourinary (GU) deformities.
- Surgical closure of the sac is ideally performed within 12 to 72 hours after birth to prevent nerve root stretching and further damage.
- Postoperative Care:
- Monitoring GU and bowel function.
- Orthopedic follow-up.
- Family education regarding home care routines, complication prevention, and bonding.
- Latex Allergy Management:
- Children with spina bifida are at high risk for serious latex allergies due to repeated exposure during multiple surgeries and lifelong urinary catheterizations.
- Goals: Reduce all latex exposure, monitor for allergic reactions, and maintain a latex-safe environment.
Muscular Dystrophies and Duchenne Muscular Dystrophy (DMD)
- General Characteristics of Muscular Dystrophies:
- A group of genetic diseases characterized by gradual muscle fiber degeneration.
- Leads to progressive weakness, skeletal muscle wasting, disability, and deformity.
- Duchenne Muscular Dystrophy (DMD):
- Also known as pseudohypertrophic muscular dystrophy.
- Epidemiology: Most common and severe form in childhood. Incidence is approximately 1 in 3500 male births.
- Inheritance: X-linked inheritance; one−third of cases are primary mutations. 60% of cases have a positive family history.
- Clinical Manifestations of DMD:

- Physical Changes: Calf muscle hypertrophy (pseudohypertrophy), lordosis, and enlarged thigh/upper arm muscles.
- Progression: Generalized weakness in adolescence leads to profound atrophy; mental deficiency is common.
- Terminal Outcomes: Death typically results from respiratory or cardiac failure.
- Diagnostic Tools for DMD:
- Polymerase chain reaction (PCR) for dystrophin gene mutation.
- Serum enzyme measurements: Serum Creatine Phosphokinase (CPK) and AST levels are extremely high in the first 2 years of life, preceding clinical weakness.
- Confirmation via Electromyography (EMG) and muscle biopsy.
- Management of DMD:
- There is no established effective treatment/cure.
- Primary Goal: Maintain function in unaffected muscles and keep the child active as long as possible.
- Interventions: Range of motion (ROM) exercises, bracing, surgical release of contractures, and genetic counseling for the family.
- Psychosocial Support: Helping the family cope with a chronic, progressive, and incapacitating disease while designing programs to afford maximal independence.
