1. Overview of the Integumentary System
  • Comprises the skin, hair, nails, and associated glands.

  • It is the largest organ system of the body.

2. Functions of the Integumentary System
  1. Protection: Acts as a physical barrier against pathogens, UV radiation, and mechanical/chemical trauma.

  2. Thermoregulation: Regulates body temperature through sweating and altering blood flow.

  3. Sensation: Contains sensory receptors for touch, pressure, pain, and temperature.

  4. Metabolic Functions: Synthesizes Vitamin D when exposed to UV radiation (7textdehydrocholesteroltextVitaminD37 text{-dehydrocholesterol} \to text{Vitamin } D_3).

  5. Excretion: Eliminates small amounts of waste products (e.g., urea, salts) through sweat.

  6. Blood Reservoir: The dermal vascular supply can hold about 5% of the body's entire blood volume.

3. Layers of the Skin (Cutaneous Membrane)
3.1. Epidermis
  • Outermost protective layer, composed of keratinized stratified squamous epithelium.

  • Avascular (lacks blood vessels).

  • Cell Types:

    • Keratinocytes: Produce keratin, a protective protein. Most abundant cells.

    • Melanocytes: Produce melanin (pigment), protecting against UV radiation. Located in the deepest layer of the epidermis.

    • Langerhans Cells (Dendritic Cells): Phagocytes, part of the immune system.

    • Merkel Cells (Tactile Cells): Sensory receptors for touch.

  • Layers (from deep to superficial) - "Come, Let's Get Sun Burned":

    • Stratum Basale (Germinativum): Deepest layer, single row of actively mitotic stem cells. Contains melanocytes and Merkel cells.

    • Stratum Spinosum: Several layers thick, cells contain pre-keratin filaments. "Prickly layer."

    • Stratum Granulosum: One to five cell layers thick. Keratinization begins; cells fill with keratin and glycolipids are released for waterproofing.

    • Stratum Lucidum: Clear layer, only found in thick skin (palms and soles). Few rows of flat, dead keratinocytes.

    • Stratum Corneum: Outermost layer, 20-30 layers of dead, anucleated cells. Provides protection and prevents water loss.

3.2. Dermis
  • Strong, flexible connective tissue layer beneath the epidermis.

  • Contains blood vessels, nerves, hair follicles, and glands.

  • Layers:

    • Papillary Layer: Superficial, areolar connective tissue with dermal papillae (finger-like projections that indent the epidermis).

      • Contains touch receptors (Meissner's corpuscles) and free nerve endings (pain receptors).

    • Reticular Layer: Deeper, dense irregular connective tissue. Contains collagen and elastic fibers, providing strength and elasticity.

      • Contains sweat glands, sebaceous glands, hair follicles, and larger blood vessels.

4. Hypodermis (Subcutaneous Tissue)
  • Not technically part of the skin, but closely associated.

  • Composed of loose areolar connective tissue and adipose tissue.

  • Functions: Anchors skin to underlying structures, insulates, and stores fat.

5. Skin Appendages
5.1. Hair and Hair Follicles
  • Hair: Flexible strands of dead, keratinized cells.

    • Functions: Protection (head, eyes), sensory detection.

  • Hair Follicle: Extends from epidermal surface into dermis.

    • Arrector Pili Muscle: Smooth muscle attached to follicle; contracts to cause "goosebumps."

5.2. Nails
  • Hard, keratinized plates that protect the distal ends of fingers and toes.

  • Structure: Nail plate, nail bed, nail matrix, cuticle (eponychium).

5.3. Glands
  • Sebaceous Glands: Oil glands, secrete sebum (lubricates skin/hair, antibacterial properties).

    • Found all over the body except palms and soles. Most empty into hair follicles.

  • Sweat (Sudoriferous) Glands:

    • Eccrine (Merocrine) Glands: Most numerous, produce true sweat for thermoregulation. ducts open directly onto skin surface.

    • Apocrine Glands: Larger, found in axillary and anogenital areas. Secretions include fats and proteins, odorless until bacterial decomposition (body odor). Ducts usually open into hair follicles. Begin functioning

6. Common Integumentary Conditions
6.1. Acne Vulgaris
  • Definition: An inflammatory condition of the sebaceous glands and hair follicles.

  • Causes:

    • Excess sebum production.

    • Keratin blockage of hair follicles.

    • Bacterial proliferation (primarily Propionibacterium acnes).

    • Inflammation.

  • Types: Comedones (blackheads/whiteheads), papules, pustules, nodules, cysts.

7. Burns
  • Definition: Tissue damage caused by heat, electricity, radiation, or chemicals.

  • Classification by Depth:

    • First-degree (Superficial): Only the epidermis is affected. Characterized by redness, mild pain, and no blistering (e.g., typical sunburn).

    • Second-degree (Partial-thickness): Involves the epidermis and part of the dermis. Presents with blisters, severe pain, and redness.

    • Third-degree (Full-thickness): Destroys the entire epidermis and dermis. The skin may appear white, cherry-red, or blackened, with little to no pain due to destroyed nerve endings.

    • Fourth-degree: Extends into underlying tissues such as muscle or bone.

  • Major Concerns: Fluid loss, infection, and impaired thermoregulation.

  • Rule of Nines: A method used to estimate the percentage of total body surface area (TBSA) affected by burns in adults, crucial for fluid replacement calculations.
