PT Interventions

Emphasis is focused on improving function for the pt to return to participation in ADLs, work and recreation

Approach to Interventions

  • moment by moment decision making throughout treatment

  • be creative but have RATIONALE behind your choice of intervention

    • if it’s too easy make it harder

Evolution of PT Interventions

  • 1920-1940s

    • muscle re-ed approaches

    • focus was on muscle strengthening and conditioning

  • 1940-1980s

    • neurofacilitation approaches

    • looking at focusing on what pt has lost and recover that function (function-based)

    • Rood, Brunnstrom, NDT, PNF

    • Rood Approach

      • 2 primary components

        • motor development sequences

        • sensory stimulation techniques (strong emphasis on sensory function)

    • Brunnstrom

      • identified:

        • abnormal synergy patterns

          • ex: if stressful, synergy will occur w/ movement

        • stages of motor recovery after stroke

    • Neurodevelopmental Treatment (NDT)

      • guides pt toward normal posture and movement patterns

      • begins w/ core stability and postural alignment

      • determine points of contact and direction of pressure

    • Proprioceptive Neuromuscular Facilitation (PNF)

      • series of specific diagonal patterns for UE and LE w/ combo of these patterns to apply to the pt during functional activities or as ther-ex

      • manual contacts are placed OPPOSITE the movement or direction of motion in order t o apply stretch and resistance

        • rotation at the hips and shoulders often lacking

        • D1

          • Flexion, adduction, ER

          • Extension, abduction, IR

        • D2

          • Flexion, abduction, ER

          • Extension, adduction, IR

  • 1980s-present

    • task-oriented approaches

    • Gentile’s taxonomy

    • regain motor function thru task related activities

Motor Relearning Program

  • task specific approach
