WHAT IS A CBC (COMPLETE BLOOD COUNT)?
A Complete Blood Count (CBC) is a test that measures various components of the blood to evaluate general health and detect a wide range of disorders, such as anemia, infection, and many other diseases.

WHITE BLOOD CELL (WBC) COUNT
Definition:
WBC refers to the white blood cell count in the blood, critical for immune response.
Normal Range:
Generally between 4.0-11.0 × 10^9 /L.
Clinical Significance:
Increased in bacterial infections.
Decreased in viral infections.
Increased in leukemias.
Types of WBC:
Neutrophils, Lymphocytes, Monocytes, Eosinophils, Basophils.
Lifespan varies: neutrophils live for hours, while memory lymphocytes can last for years. Most live only days.
RED BLOOD CELL (RBC) COUNT
Definition:
RBC refers to red blood cells, which are responsible for oxygen transport.
Normal Range:
Males: 4.7–6.1 × 10^12/L.
Females: 4.2–5.4 × 10^12/L.
Characteristics:
Also known as erythrocytes, they are bi-concave discs containing hemoglobin.
Circulate for about 120 days before being engulfed by macrophages.
HEMOGLOBIN (HGB)
Definition:
Hemoglobin is the protein in RBCs that carries oxygen and transports carbon dioxide back to the lungs.
Normal Range:
Males: 140–180 g/L.
Females: 120–160 g/L.
Clinical Significance:
Low levels indicate anemia.
HEMATOCRIT (HCT)
Definition:
HCT measures the proportion of blood volume that is occupied by RBCs.
Normal Range:
Males: 0.42–0.52.
Females: 0.37–0.47.
Quality Assurance Correlation:
Correlation checks between HGB and HCT known as the “rule of three.”
Formulas:
RBC × 3 = HGB
HGB × 3 = HCT ± 3
Example: If RBC is 9.0, then 9.0 × 3 = 27 [range 24–30].
MEAN CORPUSCULAR VOLUME (MCV)
Definition:
MCV measures the average size of red blood cells.
Normal Range:
80-100 fL.
Clinical Significance:
Low MCV indicates iron deficiency anemia, thalassemia, sideroblastic anemia, etc.
High MCV indicates megaloblastic anemia, alcoholism, B12 deficiency.
Calculation:
MCV = (HCT ÷ RBC) × 1000.
MEAN CORPUSCULAR HEMOGLOBIN (MCH)
Definition:
MCH measures the average quantity of hemoglobin in each red blood cell.
Normal Range:
27–31 pg.
Calculated in relation to MCV and MCHC, part of the RBC indices.
Calculation:
MCH = (HGB ÷ RBC).
MEAN CORPUSCULAR HEMOGLOBIN CONCENTRATION (MCHC)
Definition:
MCHC evaluates the average concentration of hemoglobin per red blood cell.
Normal Range:
320-360 g/L.
Clinical Significance:
High levels may be associated with spherocytosis, hemolytic anemia, burns, macrocytic anemias, liver disease.
False positives can occur in cold agglutinin diseases.
Calculation:
MCHC = (HGB ÷ HCT).
PLATELET COUNT (PLT)
Definition:
Measures the number of platelets in blood, essential for the clotting process.
Normal Range:
150–450 × 10^9/L.
Critical Values:
Low: 20.0 × 10^9/L
High: 1,000 × 10^9/L.
Low counts can occur in conditions such as Immune Thrombocytopenic Purpura (ITP) and Thrombotic Thrombocytopenic Purpura (TTP).
RED CELL DISTRIBUTION WIDTH (RDW)
Definition:
Measures the size and volume variation of red blood cells, indicating anisocytosis.
Normal Range:
11.5%–14.5%.
Clinical Significance:
Typically an early indicator of anemia; high RDW is seen in conditions like dual population or transfused patients.
MEAN PLATELET VOLUME (MPV)
Definition:
The average size of platelets found in blood, calculated automatically as part of the CBC.
Clinical Significance:
Increased MPV indicates the presence of larger or giant platelets, often associated with various platelet disorders.
WHITE BLOOD CELL DIFFERENTIAL COUNT
Relative Neutrophil Count (%NEUT):
Most abundant granulocyte, comprising 40-70% of WBCs.
First responders to infection.
Lymphocytes (% Lymph):
Play a vital role in the immune system, involving B and T lymphocytes.
Normal range: 20-40%, increased in viral infections.
Monocytes (% Monocytes):
Largest leukocyte, capable of differentiating into macrophages; normal range 2-8%.
Eosinophils (% Eosinophils):
Combat multicellular parasites; normal range 1-4%. High levels indicate parasitic infections or allergic reactions.
Basophils (% Basophils):
Least common granulocyte, normal range 0.5-1%. They are involved in inflammatory responses, especially in allergic reactions.
ABSOLUTE COUNTS IN WBC DIFFERENTIAL
Definition:
Absolute counts provide more accurate data than relative counts by multiplying WBC percentage by total WBC count.
Calculation Example:
If total WBC is 5.0 × 10^9/L:
Segmented Neutrophils (40%): (normal range 2.0-7.5)
Bands (3%):
Lymphocytes (55%): (normal range 1.2-3.5)
Monocytes (2%): (normal range 0-0.9)
