Physical Assessment of Body Systems

Musculoskeletal System
  • Evaluate the client's ability to perform daily living activities, especially ambulation to the bathroom for personal hygiene.

  • Assess for muscle weakness and any decrease in range of motion in limbs and joints (e.g., full extension of arms, knees, wrists).

  • Check balance and gait stability during ambulation.

  • Determine fatigue levels and overall coordination.

Skin Assessment
  • The skin is the largest organ; assess for integrity, defects, rashes, moles, lesions, and temperature.

  • Look for moisture levels and any limitations in sensation, as well as lesions, wounds, or skin ulcers.

  • Assess vascularity to ensure adequate blood flow to all extremities, ensuring proper lighting for visibility.

Oral Cavity Examination
  • Inspect teeth, lips, tongue, gums, and mucous membranes for pinkness, moisture, and the presence of lesions, bleeding, or ulcers.

  • Check for missing or loose teeth, dental caries, and the condition of any dentures (upper or lower).

  • Assess the movement of the tongue and conditions of the hard and soft palate, uvula, and tonsils for redness or edema.

Ears, Eyes, and Nasal Assessment
  • Ensure facial features are symmetrical; check for drainage, lesions, swelling, redness, or excessive tearing in eyes and ears.

  • Examine hair and scalp condition (look for hair loss, dandruff, and infestations).

Nails and Feet Inspection
  • Check the skin integrity on feet for sores or wounds, especially between toes and on heels.

  • Assess nail beds for color, consistency, and grooming adequacy (are nails cut properly?).

Perineal Area Assessment
  • Examine external genitalia for discharge, noting color and odor, along with identifying the source of discharge.

  • Assess skin for lesions, swelling, redness, or irritations, including examining the anus for any cracks, nodules, or distended veins.
