Functional Independence Measure (FIM)
-Defines the level of assistance an individual needs to grade functional status

Level 1
Total assistance (patient performs less than 25% of the effort - helper required)
Level 2
Maximal assistance (patient performs 25-49% of the effort - helper required)
Level 3
Moderate assistance (patient performs 50-74% of the effort - helper required)
Level 4
Minimal assistance (patient performs greater than 75% of the effort - helper required)
Level 5
Supervision (patient requires verbal cues, setup, or stand by assistance - helper required)
Level 6
Modified independence (patient requires assistive or adaptive devices - no helper required)
Level 7
Complete independence (no helper required)
