Oocyte Retrieval Witness
Patient Chart, 8-Well Retrieval Dish, Insem Dishe(es)
Oocyte Retrieval When
When embryologist is gets out the insem dishes (may be multiple if there are more than 20 oocytes or if split-oocyte donor) to move eggs from 8 well dish
Sperm Preparation Witness
Patient Chart, collection cup with label, all conical tubes used for sperm processing (4-15mL), sperm wash flask (frozen partner sperm needs to witness cryo vial and frozen donor needs to witness cryo vial AND donor sperm ID on stim sheet
Sperm Preparation When
Before specimen is moved from collection cup to first conical tube. IF FROZEN, witness all labeled tubes first before removing the vial from the transport tank for witnessing
Conventional Insemination Witness
Patient Chart, 15mL labeled conical tube of sperm, and insem dishes (there will be either 2 if less than 20 oocytes, or 4 if more than 20 oocytes).
IF FROZEN partner sperm, will need to witness cryo vial. If frozen donor sperm, will need to witness cryo vial and make sure that the stim lists same donor sperm ID as the vial)
If using donor oocytes, will also need to witness insem dishes LABELled with donor name, social security number, and donor ID, and verify that recipient stim sheet lists same oocyte donor ID from dish
